Thank you for being a facilitator for a Rotary Community Group (RCG) establishment meeting! You've played an important role in making this new means of collaboration between clubs possible.

Please read the following carefully before completing this form:

• This form should be submitted on behalf of an RCG by the person who has facilitated their establishment meeting.
• Please ensure you enter the RCG's official code into this form. Each RCG's official code can be found in the spreadsheet outlining the full list of RCGs, and is featured in the underlined blue text above the list of clubs in an RCG. Examples of official codes include 9423-1, 9780-3, and 9910/20-5.
• If you are facilitating establishment meetings for multiple RCGs, please submit a separate form for each RCG.
