Use this form to nominate an Interact club for the Club Excellence award. The person who completes the form needs to be an officer of the Interact club’s sponsor club or a district officer.

You'll need information about the Interact club, including its complete name and ID number, its sponsor club’s name and ID number, the district number, and the district governor's name and email address. You'll also need to know which goals the club achieved. 

If you're nominating more than one Interact club, use a separate form for each one.

All nominations need to be submitted by 15 August.
